The Risk Management and Insurance Department administers all claims under the University's policies and through its self-insurance program and negotiates claims settlement with its various insurers.
All claims should be reported as soon as they are discovered so that facts can be determined as quickly and as accurately as possible. Timely reporting of losses benefits the University as the severity of the loss can often be mitigated if action is taken as soon as practicable. All UofT insurance policies require that losses be reported immediately.
In the event of any loss (personal injury, property damage, automobile, etc.) the department involved must:
- File a report either with Campus Police (416) 978-2323 or the local police department.
- Report all claims immediately to Tanya V. Patina, Insurance and Claims Administrator.
- Make no admissions, settlements or offers, or sign any statements except as required by a police officer.
GUIDELINES FOR REPORTING A CLAIM
Automobile Accidents (University owned or leased vehicles)
- Contact the UofT Campus Police at (416) 978-2222 if personal injuries involved and at (416) 978-2323 if only property damage involved, so that an accident report can be filed. The Campus Police will contact Toronto Police or Ambulance if necessary.
- Get officer’s name, badge number, police division number, and police occurrence number.
- Do not admit liability.
- Obtain names, addresses and insurance information of the other driver/drivers and owner/owners of the car/cars involved in the accident.
- Obtain names and contact information of any witnesses.
- Get names and addresses of the injured people, and note kind of injuries.
- If the accident is minor in nature, visit the nearest Collision Reporting Centre to file an accident report. This should be done within 24 hours of the accident, especially in the case of a hit-and-run as the deductible may be waived in some cases.
- Notify the Risk Management and Insurance Department, Tanya V. Patina, Insurance and Claims Administrator.
- Complete the Driver’s Report of Accident form and send it to the Risk Management and Insurance Department.

Damage to U of T Property
Staff are encouraged to document all serial numbers and models of all valuable equipment (for example, LCD projectors, laptops, etc.). In cases of a break and enter do not allow anybody to enter the scene to avoid contamination.
- Immediately contact the UofT Campus Police at (416) 978-2323 so that an accident report can be filed.
- In case of break-in or theft the UofT Campus Police will notify Toronto Police if necessary.
- Prepare a Property Damage Incident Report for the Risk Management and Insurance Department.
Incident Date, Time and Location
- Date and time of the incident.
- Describe the area and location where this incident occurred.
- Who discovered the theft/damage/etc.
- Did anyone witness anything or anyone strange in the area prior to the theft/damage/etc. occurring?
- What valuables and special equipment were in the office?
- Describe any signs of forced entry.
- Describe any damage to the building.
- Was there any broken glass.
- Did the glass fall inside or outside.
- What employees have access to the area where the incident occurred?
- Was the business locked when this incident occurred?
- Describe any alarms that are located on the property.
Police investigation
- Was the Campus Police called to the scene?
- Who called the Campus Police?
- Who investigated? Get officer’s name, badge number, police division number, and police occurrence number.
- Were any photographs taken of the scene?
- Previous occurrences.

When the loss is such that repairs must be handled immediately (i.e., fire, flood, large number of computer losses, etc.) please contact the Risk Management and Insurance Department directly at (416) 978-7484. We will notify the UofT insurance carrier to initiate the claims process.
